For your convenience, this document will appear in a separate window and you can fill in the blanks on line. Because of limitations with Adobe Acrobat Reader, you cannot save the document. So here is what to do next:- Print a copy of the filled in form for you records.
- Fax that SIGNED copy to XSBaggage.
- If you use the snapshot button on the Acrobat Reader (the little
camera), you can drag your mouse over the filled in pdf form. So, open up your mail program, go back to the pdf file, take a snapshot of your filled in form, paste that into your email and send that to XSBaggage as well (if you can ... if you can't figure this out, it's okay. We'll work around it.)
- Goods are accepted and carried subject to our terms

- Your shipment is also subject to a Dangerous Goods statement. Print this off and sign it.

To cover our credit card and bank fees associated with receiving large payments, rather than increase the quoted costs for all our clients, it is our company policy that only invoices greater than USD 700.00 will incur the credit card surcharge of 2.5% we must pass along to our clients to cover our costs associated with receiving payment in this manner.
Alternatively, payment for invoices over USD 700.00 may be made by bank transfer/wire directly into our account, and will incur only a flat $25 bank fee, which is our cost to receive inbound wire transfers, irrespective of the amount of the invoice.
If your total exceeds USD 700.00, please let us know when you provide us with your credit card details if you prefer to pay by bank transfer instead of by credit card. If we dont hear from you, we will proceed to process payment using the credit card provided, applying any applicable fees as above.
*Note: A United States billing address must be provided along with credit card details for payment. Overseas cards without a United State billing address will be charged an additional 1% cross-border credit card transaction fee, in addition to any other applicable fees as outlined above, to cover our costs associated with accepting foreign credit cards. All government, corporate, and business credit cards will incur a non-qualified card transaction fee of 1%, in addition to any other applicable fees as outlined above.
